IN PERSON:

Student Self-Request
  1. Ask the student's name and BHSU ID number.
  2. Verify the student’s valid photo ID.
    1. Electronic BHSU ID is acceptable as valid ID.
Third Party Request (Parents, spouses, friends, employers, classmates, vendors, etc.)
  1. Ask the student’s name and BHSU ID number.
  2. Verify that the third party is listed on a FERPA Request, Authorization, and Consent Form (SPACMNT).
  3. Verify third party’s identity with a valid government photo ID.

Student Self-Request
  1. Ask the student’s name and BHSU ID number.
  2. Ask identifying questions (suggestions below) until you are comfortable that you have confirmed the student’s identity.
Third Party Request
  1. Ask the student’s name and BHSU ID number.
  2. Verify that the third party is listed on a FERPA Request, Authorization, and Consent Form (SPACMNT).

 

EMAIL:

University personnel should only use their official BHSU email when corresponding with students or third parties.

Student Self-Request with personal email
  1. Email should be sent from yellowjacket email.
  2. Have student include full name and student ID number in their written request.
    1. Avoid sending other personally identifiable information via email.
Third Party Request with personal email
  1. Ask for student’s full name and BHSU ID# number.
  2. Verify that the third party is listed on a FERPA Request, Authorization, and Consent Form (SPACMNT).
  3. CC email response to the student’s yellowjacket email account.
