Resolution
BHSU Students may fill out the forms below to make administrative based requests to their account. All completed forms must be submitted to the Admissions Office, and please keep in mind that it may take up to a few business days for the request to be processed.
- Family Educational Rights and Privacy Act (FERPA) form
- FERPA is a federal law that protects your educational records as a student, and a form must be submitted if you'd like to grant anyone else access to your records at BHSU.
- Immunization waiver exemption form
- The Immunization waiver exemption form is available to students that would like to request exemption from vaccine requirements.
- Student Legal Name Change form
- The Student Legal Name Change form is available to Students that have had their name legally changes since initially applying to BHSU. This helps update your account information to align with your new legal name.
- Student Preferred Name form
- The Student Preferred Name form is available to Students that would like to request a preferred name/nickname be used for their BHSU account instead of their legal name.
- Residency appeal for tuition purposes form
- The Residency appeal for tuition purposes form allows students, who're residents of SD, to request that their tuition rate be adjusted to reflect their residency.
